If your receipts are not available to attach, please submit them to Student Engagement Officer, Sarah, in Room 490.2.060 or to the Guild receptionist as soon as you have submitted this form.
We will not be able to refund your club unless we have received the receipts.
Please ensure:
- Your receipts are CLEARLY LABELLED ie. Soccer Club - Thanksgiving dinner 13/4/17, Music Club - Keyboard purchase etc.
- Please send through the receipts for the same event in one file. Ie. All receipts from Thanksgiving dinner are uploaded below as 1 file. If you have more than 4 occasions, simply merge 2 events into one file upload, just ensure the receipts are clearly labelled.
